Tips for completing the form

1. Ensure you have evidence to substantiate your claim. This can be a document, letter, audio/video recording or a signed ” Testimony in the form of an Affidavit”.
2. Please try and provide an email address of the perpetrators so they are advised that they have been publicly gazetted.
3. If there is more than one, click the “+” button  to the email address to add more fields.
4. If you need to save and come back to the form, click “save and continue” and follow the instructions.
5. When you have finished click the submit button. Your report will automatically populate the gazette for the public record. 

Note: You will receive a copy of your report after you have submitted it.
